Gout is a Western medical diagnosis, but Traditional Chinese Medicine also recognizes gout, though it refers to rheumatoid arthritis and suppurative arthritis. In this case, the patient's right big toe joint initially showed redness, swelling, heat, and pain, later spreading to the right ankle and knee joints. Serum uric acid levels were between 372 and 441 µmol/L. The formula used Cangzhu 15 grams, Huangbo 12 grams, Sheng Yiren 30 grams, Ni Xi 12 grams, Papaya 12 grams, Zhi Mu 9 grams, Huashi 30 grams, Jixueteng 30 grams, Danggui 15 grams, Chishao 15 grams, Bixie 12 grams, and Qingdai 6 grams. Boil in water and take once daily. After 12 doses, add Mutong and Silu Luo, totaling 18 doses, and the disease was cured. Although this formula is essentially a modified version of Simiao San, its composition is largely similar to Bixie Shen Shi Tang. Bixie Shen Shi Tang consists of Bixie, Sheng Yiren, Huangbo, Chishao, Danpi, Zexie, Huashi, and Mutong. This formula originates from Gao Jinting's "Collection of Insights on Ulcer Diseases," and clinically can treat: ① foot dampness; ② erysipelas; ③ acute eczema; ④ urinary tract infection; ⑤ damp tropical discharge; ⑥ impetigo; ⑦ nodular erythema of the lower limbs.
