Treatment of Arrhythmias, January 22, 1992

“The National Compendium of Famous Prescriptions” lists two formulas: one is “Jianzhong Fumai Tang,” which is essentially Da Jianzhong Tang plus 15 g of Dan Shen, 15 g of Ku Shen, and 30 g of Yu Zhu. Based on the cases presented, this formula is suitable for arrhythmias associated with sick sinus syndrome. Adding Shengmai San, Sheng Long Gu, Oyster, Huangqi, Sheng Di Huang, Xuan Shen, Mai Dong, and Ma Huang Fu Zi Xi Xin Tang to this formula may further enhance its effectiveness. The other is “Ningxin Yin,” whose composition is Shengmai San combined with Gan Mai Da Zao, plus Sheng Long Gu, Sheng Oyster, Dan Shen, Baihe, and Magnetite. According to the cases presented, one case of supraventricular tachycardia was cured by this formula. The former inherits the tradition of Zhi Gan Cao Tang and creatively promotes Da Jianzhong Tang, with Yu Zhu and Sheng Di Huang as its highlights. The latter inherits Shengmai San combined with Gan Mai Da Zao, still using Dan Shen, Ku Shen, and Baihe, while adding stone-based sedatives.

In summary, the treatment of arrhythmias should balance Zhi Gan Cao, Da Jianzhong, Shengmai San, Gan Mai Da Zao, with adjustments based on Dan Shen, Sheng Di Huang, Ku Shen, Yu Zhu, and Baihe. Notably, this approach emphasizes yin-nourishing herbs, with Sheng Di Huang, Yu Zhu, and Baihe being commonly used. Previously, Wang Lijuan’s arrhythmia was treated with Gui Pi Tang combined with Shengmai San, plus Gualou and Xie Bai, with heavy use of Sheng Di Huang, Bu Gu Zhi, Ze Xie, and Mu Tong yielding good results. Additionally, the following considerations apply to arrhythmia treatment: ① Multifocal premature beats: Dan Shen, Ku Shen, Gan Song, Dang Gui, Sangjisheng, Huangqi, and Ge Gen. ② Sinus tachycardia: Cangzhu. ③ Sick sinus syndrome: Ma Huang, Xi Xin, Fu Pian, and Bu Gu Zhi. ④ Myocarditis: Ban Xia and Yin Yang Huo. ⑤ Keshan disease: Shanzha and Wu Wei Zi. ⑥ Hypotension: Huang Jing, Zhike, and Nu Zhen Zi.
