There are quite a few ordinary people who suffer from hypotension. In ancient times, since blood pressure was not known, people relied on the six pulses being deep and fine as evidence, thus diagnosing conditions such as "deficiency of both heart and spleen," "collapse of middle qi," and "loss of yang or yin." The main symptoms of this condition include dizziness, headache, palpitations, forgetfulness, insomnia with frequent dreams, sallow complexion, poor appetite, fatigue, and spontaneous sweating. Yan Yunhe used Guipi Tang, Li Dongyuan used Buzhong Yiqi Tang, and Zhang Xichun used Shengxian Tang; if it progresses to severe yang deficiency with profuse sweating and cold extremities, then Zhongjing's Sini Tang is used. In my experience, combining Guipi Tang with Shengmai San yields the best results; for individual patients with good sleep, I substitute Buzhong Yiqi Tang for Guipi Tang. Zhang Xichun said: when the vital energy sinks, one experiences shortness of breath, palpitations, and falls; he believed that Shengxian Tang could achieve complete efficacy. This formula consists of Shengma, Huangqi, Chaihu, Zhimu, and Jiegeng. I often add Zhimu, Jiegeng, Shengma, and Chaihu to Guipi Tang, which often doubles the effect. Patients with hypotension often have poor coronary circulation due to low blood pressure, leading to symptoms similar to those of coronary heart disease, such as precordial pain and palpitations. Therefore, I add Guan II, Zhishi, Bugu Zhi, Digupi, Zexie, Mutong, Qingchenpi, Shenglonggu, and Muli to the previous formulas, naming the resulting decoction Fufang Guipi Tang, with the mnemonic: "Guipi, Guan, Shengmai, Qinglong breaks earthwater."
