Tienam is currently another antibiotic with good clinical efficacy following cefoperazone and ceftriaxone. Its main active ingredient, imipenem, is a carbapenem antibiotic, which differs from penicillin and cephalosporins in that it has stronger stability against β-lactamase, effectively combating β-lactamase. Another component of Tienam, cilastatin, effectively blocks the metabolism of imipenem in the kidneys, allowing it to be excreted in its original form, making it highly effective for urinary tract infections. However, Tienam also puts pressure on the kidneys. Tienam is suitable for all respiratory, digestive, and urinary tract infections, but the dosage should be reduced for urinary tract diseases. The usual dosage is 1–4g, administered once or twice daily via intravenous infusion, preferably slowly—at least 40 minutes per gram—so it is recommended to dilute it in 300mL or more of 0.9% sodium chloride solution (30 drops per minute).
