Answer: Your TSH level is quite low, so hyperthyroidism cannot be ruled out. However, the significant elevation of your transaminases and alkaline phosphatase suggests cholestatic liver damage. If the spleen thickness exceeds 4 cm and the portal vein diameter is greater than 13 mm, it can be diagnosed as cholestatic cirrhosis. If there is no hepatitis B or C, it may be autoimmune cirrhosis, often accompanied by multi-organ damage. Low TSH can also be explained by autoimmune diseases. Treatment should focus on liver disease first; once the liver condition improves, hyperthyroidism can be further considered.
