Second: Changes in Preload and Postload During Heart Failure
Heart failure occurs when the heart’s ejection volume cannot meet the body’s needs. In such cases, both preload and postload change. The key feature is increased preload and increased postload: preload increases because the ventricular capacity at the end of diastole increases (due to the accumulation of blood that was not yet ejected), while postload increases due to reflexive constriction of systemic arteries in an attempt to improve overall blood supply—effectively increasing the heart’s postload.
