Yang Deficiency with Water Excess

Symptoms include fatigue, exhaustion, abdominal distension, ascites, lower limb edema, chills, spontaneous sweating, deep and fine pulse, plump tongue body, and pale coating.

The formula used is Compound Liu Junzi Tang, decocted in water and taken once daily.

Liver Function: Jaundice index 4 units, TFT++, TTT 17 units, GPT over 500 units. Total plasma protein 5.8g, albumin 2.9g, globulin 2.9g. HBsAg 1:256, Anti-HBc positive.

Western Medical Diagnosis: Viral hepatitis, Type B, chronic (active).

TCM Syndrome Differentiation: Patient experiences pain on both sides of the waist, fullness and distension in the upper abdomen, dull complexion, hepatosplenomegaly, string-like rapid pulse, red tongue with ecchymosis, and yellow greasy coating. The syndrome is characterized by blood stasis and heat accumulation, which has long-term stagnation leading to fire.

Prescription: Compound Yi Yin Decoction with added ingredients

After taking 10 doses, pain on both sides of the waist and abdominal distension both eased, the complexion became slightly redder, the pulse remained string-like, the tongue still showed ecchymosis, and the yellow greasy coating became thinner. Then, 10g of Prunella vulgaris was added, followed by another 30 doses.

Follow-up Visit: All symptoms have subsided, ecchymosis on the tongue has disappeared, the complexion has turned redder, liver function shows TFT+, TTT 6 units, GPT 80 units, HBsAg (-), Anti-HBc negative. Advised to take Xiaoyao Wan twice daily, one pill each time. One month later, liver function tests showed all indicators within normal range.


Clinical Application in the Middle Section

Follow-up Visit: Symptoms have greatly improved, but still feel weak, palpitate, have a deep and fine pulse, red tongue, and slightly yellow greasy coating. Removed Guazhou from the previous formula and added 3g of Coptis chinensis, followed by another 20 doses.

Follow-up Visit: All symptoms have completely subsided, TTT 8 units, TFT (-), GPT 120 units, HBsAg 1:16, Anti-HBc positive. Advised to take Guipi Wan twice daily, one pill in the morning and one in the evening, and to recheck liver function, HBsAg, and Anti-HBc after two months, all of which will be normal.
