1. Treatment of Organ Deficiency Syndromes
(1) Lung-Kidney Type
The lung and kidney have a mutually generating relationship; lung deficiency can lead to kidney deficiency. Lung-kidney yang deficiency often indicates reduced organ function, while lung-kidney yin deficiency often suggests hyperactive organ function. In peripheral blood, the white blood cell count in the former is usually low, whereas in the latter it tends to increase. However, there is no significant difference in bone marrow morphology between the two.
① Lung-Kidney Yin Deficiency
[Main Symptoms] Dizziness, fatigue, insomnia, frequent dreams, fever—especially in the afternoon—flushed cheeks, spontaneous sweating and night sweats, weak voice, physical weakness, red tongue, and fine, rapid pulse.
[Treatment Principle] Tonify qi and nourish yin.
[Prescription] Qi-Nourishing and Yin-Tonifying Decoction: 30 grams of northern ginseng root, 30 grams of Lu Dangshen, 15 grams of Huai Shan Yao, 12 grams of ginseng roots, 9 grams of Ophiopogon japonicus, 30 grams of floating wheat, 30 grams of Cornus officinalis, 30 grams of calcined oyster shell, 30 grams of Schisandra chinensis, 9 grams of Ziziphus jujuba seeds, 9 grams of honey-fried licorice, 15 grams of calcined dragon bone, 24 grams of Rehmannia glutinosa, and 1 jujube fruit.
② Lung-Kidney Yang Deficiency
[Main Symptoms] Pale complexion, lower back and spine soreness, hearing loss, frequent and clear urination, shortness of breath and reluctance to speak, aversion to cold and excessive sweating, poor vision, pale tongue, thin white coating, and deep, fine pulse.
[Treatment Principle] Tonify qi and assist yang.
[Prescription] Qi-Tonifying and Yang-Assisting Decoction: 9 grams of Jilin ginseng roots, 30 grams of Lu Dangshen, 13 grams of Taizi Shen, 12 grams of Ophiopogon japonicus, 5 grams of Schisandra chinensis, 13 grams of goji berries, 24 grams of Cornus officinalis, 24 grams of Rehmannia glutinosa, 18 grams of Polygonum multiflorum, 9 grams of Psoralea corylifolia, 24 grams of Cistanche deserticola, 12 grams of Morinda citrifolia, 9 grams of donkey-hide gelatin beads, 9 grams of deer antler glue, 18 grams of Millettia reticulata, 3 slices of fresh ginger, and 5 jujubes.
(2) Lung-Kidney-Liver Type
On the basis of lung-kidney yang deficiency, if the liver yang is inherently deficient and water fails to nourish wood, then lung-kidney yin deficiency and liver yin deficiency will occur; if the liver yang is inherently strong, then the yang excess will become even more severe, potentially transforming into fire and wind, resulting in internal liver wind.
① Lung-Kidney Yin Deficiency, Liver Yin Deficiency
[Main Symptoms] Shortness of breath and reluctance to speak, dizziness, tinnitus, dry eyes, blurred vision, irritability, lower back and leg pain, or numbness and difficulty in bending and stretching limbs, low-grade fever, heat in the palms and soles, especially in the afternoon, women may experience scanty menstruation or amenorrhea, thirst and desire to drink, constipation, and a fine, rapid pulse with a string-like quality, along with a red tongue and sparse coating. Blood tests show elevated white blood cells but decreased other blood parameters.
[Treatment Principle] Tonify qi, nourish yin, and clear the liver.
[Prescription] Modified Shengmai Di Huang Tang: 15 grams of ginseng root, 15 grams of Ophiopogon japonicus, 6 grams of Schisandra chinensis, 18 grams of Rehmannia glutinosa, 24 grams of Huai Shan Yao, 15 grams of Cornus officinalis, 9 grams of Poria cocos, 9 grams of Alisma plantago-aquatica, 6 grams of Cortex Moutan, 9 grams of processed Polygonum multiflorum, 9 grams of goji berries, 15 grams of turtle shell glue, 15 grams of live soft-shelled turtle, 9 grams of Ligustrum lucidum, 9 grams of Eclipta prostrata, 9 grams of wild lily, 24 grams of Astragalus membranaceus, 9 grams of Angelica sinensis, 6 grams of Jilin ginseng roots, 9 grams of Taizi Shen, 9 grams of Dangshen, 9 grams of Artemisia annua, 9 grams of fried Chaihu with turtle blood, and 6 grams of raw licorice.
② Lung-Kidney Yin Deficiency, Internal Liver Wind
[Main Symptoms] Shortness of breath and cough, lower back and leg pain, tidal fever, dry mouth, nocturnal emission, night sweats, emaciation, headache, dizziness, and in severe cases, mental confusion, facial and eye deviation, stiff neck, symptoms of meningeal leukemia, weak pulse, and a dark red tongue with little coating.
[Treatment Principle] Nourish yin, clear the lungs, and calm the liver to extinguish wind.
[Prescription] Modified Da Ding Feng Zhu: 15 grams of ginseng root, 15 grams of Dangshen, 9 grams of Jilin ginseng roots, 9 grams of Ophiopogon japonicus, 4.5 grams of Schisandra chinensis, 15 grams of Huai Shan Yao, 12 grams of Ziziphus jujuba seeds, 12 grams of raw white peony, 9 grams of dried Rehmannia glutinosa, 9 grams of donkey-hide gelatin beads (dissolved), 12 grams of live turtle shell, 15 grams of live oyster shell, 15 grams of hemp seeds, decocted and strained, mixed with 2 egg yolks, 3 grams of scorpions, and 3 grams of centipedes (separately ground) for oral administration. For coma, use Angong Niuhuang Wan.
(3) Lung-Kidney-Liver-Spleen Type
When the lungs are deficient and metal fails to generate water, the kidneys become deficient; since the liver and kidneys share a common origin, kidney disease leads to liver disease, which can further result in the pathological mechanism of liver wood overcoming earth, thus giving rise to this syndrome type.

[Main Symptoms] Shortness of breath and reluctance to speak, dizziness and vertigo, lower back and leg pain, fullness and distension in both flanks, discomfort in the stomach, belching and acid regurgitation, lack of appetite, irritability, enlarged liver and spleen, loose stools, and a string-like, rapid pulse with no sharp edges, along with a white, greasy coating on the tongue.
[Treatment Principle] Tonify qi and nourish yin, soothe the liver and strengthen the spleen.
[Prescription] Modified Xiaoyao San: 9 grams of fried Chaihu with turtle blood, 9 grams of Angelica sinensis, 9 grams of white peony, 9 grams of Poria cocos, 9 grams of Atractylodes macrocephala, 9 grams of honey-fried licorice, 15 grams of Dangshen, 9 grams of Ophiopogon japonicus, 4.5 grams of Schisandra chinensis, 15 grams of Astragalus membranaceus, 15 grams of Cornus officinalis, 9 grams of goji berries, 15 grams of Huai Shan Yao, 9 grams of roasted hawthorn, 9 grams of fermented malt, 9 grams of polygonatum, and 9 grams of yellow gentian. For those with enlarged liver and spleen, add Danshen, pangolin scales, seashells, peach kernels, and safflower; for those with enlarged lymph nodes, add Xiao Jin Dan (made from white frankincense, processed Aconitum carmichaelii, five spirit resins, dried Rehmannia glutinosa, earthworms, myrrh, frankincense, angelica, and musk), 3 pills each time.
② Lung-Kidney Liver Deficiency, Spleen Unable to Control Blood
[Main Symptoms] Pale complexion, weak voice, occasional spontaneous sweating, clear urine and loose stools, chronic bleeding over a long period, palpitations, cold hands and feet, bland taste, poor appetite, weak pulse, and a white, moist tongue.
[Treatment Principle] Tonify qi and nourish yin, strengthen the spleen and stop bleeding.
[Prescription] Modified Gui Pi Tang: 9 grams of Jilin ginseng (decocted separately), 9 grams of Atractylodes macrocephala, 9 grams of Poria cocos, 3 grams of patchouli, 9 grams of Ziziphus jujuba seeds, 9 grams of longan pulp, 15 grams of Millettia reticulata, 18 grams of Astragalus membranaceus, 9 grams of Angelica sinensis, 9 grams of Taizi Shen, 9 grams of Eclipta prostrata, 9 grams of Ligustrum lucidum, 9 grams of deer antler glue, 9 grams of donkey-hide gelatin beads, 9 grams of Huai Shan Yao, 9 grams of yellow gentian, 15 grams of white grass root, 6 grams of honey-fried licorice. For heavy menstrual bleeding, add 15 grams each of large thistle and small thistle, 15 grams of carbonized earth, 15 grams of carbonized palm leaves, 15 grams of calcined dragon bone, 15 grams of calcined oyster shell, and 15 grams of immortelle herb; for hematuria, add 18 grams of white grass root and 9 grams of purple leaf herb.
(4) Five Organs Dysregulation Type
If lung-kidney yang deficiency occurs first, followed by liver-spleen deficiency, it will inevitably affect the heart, which governs all five organs. Liver deficiency means wood cannot generate, spleen deficiency means the child steals the mother's qi, and kidney water deficiency means it cannot ascend to the heart, disrupting the balance of water and fire. The heart governs blood, while the lungs govern qi; when qi is deficient, blood is also difficult to replenish, leading to a situation where all five organs are deficient, with the heart being the most severely affected.
① Five Organs Dysregulation, Cold Deficiency in the Heart
[Main Symptoms] Palpitations, shortness of breath, spontaneous sweating, fatigue and weakness, pale complexion, cold limbs, profuse sweating, poor appetite, confusion, blood counts below normal, extremely weak pulse, red tongue with no coating or thin white coating, commonly seen in advanced stages of leukemia.
[Treatment Principle] Tonify qi and nourish the heart, vigorously supplement heart yang.
[Prescription] Da Bu Hui Yang Tang: 30 grams of honey-fried Astragalus membranaceus, 24 grams of Dangshen, 15 grams of Taizi Shen, 9 grams of Jilin ginseng, 6 grams of dried Aconitum carmichaelii, 9 grams of Atractylodes macrocephala, 9 grams of Poria cocos, 6 grams of honey-fried licorice, 9 grams of Angelica sinensis, 4.5 grams of cinnamon, 12 grams of Rehmannia glutinosa, 9 grams of white peony, 6 grams of Chuanxiong, 12 grams of mountain yellow meat, 15 grams of Huai Shan Yao, 3 grams of dried ginger, 2.4 grams of deer antler (ground finely), 9 grams of donkey-hide gelatin, 24 grams of Millettia reticulata, 24 grams of mulberry fruits, 9 grams of Psoralea corylifolia, 9 grams of Schisandra chinensis, 9 grams of ginger, 3 jujubes.
② Five Organs Dysregulation, Yin Deficiency and Yang Collapse
[Main Symptoms] Fever, profuse sweating, difficulty in continuing breathing, loss of appetite, confusion, both white blood cells and red blood cells are low (or related to chemotherapy), weak pulse with a string-like quality.
[Treatment Principle] Nourish yin and raise yang.
[Prescription] Sheng Yang Jiu Yin Tang: 15 grams of ginseng root, 12 grams of Xuan Shen, 24 grams of Dangshen, 9 grams of sugar-coated ginseng, 9 grams of Taizi Shen, 15 grams of Danshen, 9 grams of Ophiopogon japonicus, 6 grams of Schisandra chinensis, 24 grams of Cornus officinalis, 15 grams of goji berries, 30 grams of Huai Shan Yao, 30 grams of Rehmannia glutinosa, 12 grams of Angelica sinensis, 9 grams of white peony, 12 grams of dried Rehmannia glutinosa, 15 grams of dragon bone, 45 grams of floating wheat, 9 grams of Atractylodes macrocephala, 9 grams of Qin Jiao, 15 grams of donkey-hide gelatin, 9 grams of white weeping, 9 grams of green artemisia, 9 grams of turtle shell, 30 grams of Millettia reticulata, 9 grams of Poria cocos, 6 grams of honey-fried licorice, 9 grams of焦楂, 9 grams of神曲, 3 grams of Chaihu, 3 grams of Shengma, and 5 jujubes.
2. Treatment of Excessive Pathogenic Factors
(1) Warm Disease Type
Leukemia (especially acute leukemia) often presents with signs of wind-heat and real-fire syndromes. Sometimes there is headache, fever, red tongue, and rapid pulse, indicating heat in the defensive layer; sometimes there is high fever, thirst, profuse sweating, and constipation, indicating heat in the qi layer; sometimes there is blood heat running rampant, with vomiting, nosebleeds, and bloody stools, indicating heat in the nutritive layer. In general, since this disease exhibits characteristics of warm diseases, medications should be selected that clear heat and detoxify, such as honeysuckle, Forsythia suspensa, peppermint, dandelion, Isatis tinctoria, Viola philippica, seven-leaf-one-flower, mulberry leaves, and so on.
① If toxic heat attacks the lungs and causes throat swelling and pain, use Scutellaria baicalensis, Sophora flavescens, Arctium lappa, Herba Bistorta, raw grass, Xuancao, and White Snake Tongue Herb.
② If phlegm cough is not relieved, use Houttuynia cordata, Fritillaria cirrhosa, Mulberry bark, raw gypsum, or Ma Xing Shi Gan Tang.
③ If lung heat causes nasal bleeding, use White Grass Root, Cortex Moutan, Rehmannia glutinosa, charred gardenia, and madder root.
④ If lung heat spreads to the large intestine, causing constipation, use Rhubarb and Glauber's salt; if damp-heat causes diarrhea, use Coptis chinensis, Scutellaria baicalensis, Pulsatilla chinensis, Qin Jiao, and purslane.
⑤ If toxic heat attacks the kidneys, causing reddish urine and turbid discharge, use Anemarrhena asphodeloides and Phellodendron amurense; if kidney heat spreads to the bladder, causing damp-heat to descend and resulting in reduced urine output and urinary tract infections, use Gardenia jasminoides, Setaria viridis, Plantago asiatica, Talc, Solanum nigrum, and Semen Lonicerae.
⑥ If liver and gallbladder have real-fire, with red eyes and bitter mouth, use Gentiana scabra and Gardenia jasminoides; if liver heat spreads to the gallbladder, causing jaundice, use Artemisia capillaris and Phellodendron amurense.
⑦ If toxic heat attacks the heart, causing excessive heart fire and irritability, use Coptis chinensis, Folium Mori, and Forsythia suspensa; if heart heat spreads to the small intestine, causing red and painful urine, use Rehmannia glutinosa, Mu Tong, and raw licorice; if skin develops widespread boils and pimples, heavily use honeysuckle, Forsythia suspensa, dandelion, and Viola philippica; if sepsis occurs, use Coptis chinensis detoxification soup with added ingredients.
⑧ If heat enters the nutritive layer, depleting the nutritive blood, causing high fever at night, faint rashes, fine and rapid pulse, and a dark red tongue with no coating, use rhinoceros horn, Rehmannia glutinosa, Xuan Shen, bamboo leaf rolls, Ophiopogon japonicus, Danshen, Coptis chinensis, honeysuckle, Forsythia suspensa, and other herbs.
⑨ If heat enters the pericardium, causing high fever, delirium, and speech disorders, accompanied by phlegm heat, with a slippery and rapid pulse, due to internal heat trapping, high fever is often accompanied by cold extremities, so it is necessary to clear the heart and relieve heat to reverse the trend. Medications such as Angong Niuhuang Wan, Zhibao Dan, and Zixue Dan can be used.
⑩ If pathogenic heat remains in the yin layer and does not dissipate, with low-grade fever that fluctuates between light and heavy, heat in the palms and soles, and a red tongue with a fine, rapid pulse, use Qin Jiao, turtle shell, Rehmannia glutinosa, Anemarrhena asphodeloides, Diguopi, Artemisia capillaris, fried Chaihu with turtle blood, white weeping, Hu Huang Lian, and turtle shell.
If toxic heat attacks the spleen, causing dry mouth and lips, easy thirst and hunger, use gardenia, raw gypsum, and raw grass; if spleen heat affects the stomach, causing dryness and hardness in the middle jiao, red eyes and thirst, abdominal fullness and constipation, use Rhubarb and Glauber's salt or Liangge San with added ingredients.
In addition to the above methods of clearing heat and detoxifying, for some leukemia patients who frequently experience generalized joint pain, obvious tenderness in the sternum, hoarseness, astringent pulse, or combined low-grade and high-grade fevers that do not subside, treatment should be based on blood stasis, using Sanleng, Wo Shu, Huang Yao Zi, Jing Tian San Qi, She Liu Gu, and Shan Ci Gu.
(2) Five Organs Interacting, Real Fire in the Liver Type
[Main Symptoms] Chest tightness and weakness, chest pain or sternum pain, bitter taste, tinnitus, irritability, insomnia, abdominal masses that are hard and do not disappear, a string-like, powerful pulse, often with a red tongue and even purple-dark spots.
[Treatment Principle] Clear heat, drain the liver, and eliminate accumulation.
[Prescription]: Modified Danggui Aloe Decoction: 9 grams of Angelica sinensis, 9 grams of aloe vera, 9 grams of Gentiana scabra, 6 grams of Coptis chinensis, 6 grams of Scutellaria baicalensis, 6 grams of Phellodendron amurense, 6 grams of Gardenia jasminoides, 6 grams of Rhubarb, 6 grams of Qing Dai (packaged), 3 grams of patchouli, 0.15 grams of musk, 30 grams of White Snake Tongue Herb, 15 grams of Half Branch Lotus, 24 grams of live oyster shell, 24 grams of live turtle shell, for those with enlarged liver and spleen that do not shrink, add 6 grams of Sanleng and 6 grams of Wo Shu.
III. Case Introductions
[Case 1] Ms. Gou, 25 years old, a villager. March 16, 1974. First visit. For several months, she has been feeling listless, with poor appetite that gradually worsens, fatigue, palpitations, shortness of breath, insomnia and frequent dreams, fever (especially in the afternoon), heat in the palms and soles, frequent night sweats, early menstruation, reduced menstrual flow, lower back and leg pain, and occasional dizziness. She had previously undergone examination at the affiliated hospital of a medical college, where her white blood cell count was 210,000/mm³, with enlarged liver and spleen, and was diagnosed with chronic myelogenous leukemia. On examination, her inner cheeks were flushed, her voice was low, she felt dizzy and weak, and other symptoms were as described above. The tip of her tongue was slightly red, and her pulse was fine and rapid. Based on the lung-kidney yin deficiency type, the prescription was Qi-Nourishing and Yin-Tonifying Decoction with 24 grams of White Snake Tongue Herb, 15 grams of Half Branch Lotus, and 3 grams of Qing Dai (packaged) for clearing heat and detoxifying; plus 15 grams of Danshen, 9 grams of Guang Yu Jin, 18 grams of live oyster shell, and 18 grams of live turtle shell for softening the liver and reducing the spleen. After taking the above prescription for two months, her spirits, sleep, and appetite all returned to normal, and she was able to engage in light physical labor, continuing to take the original prescription. On April 6, 1975, during the follow-up visit, her menstruation was delayed by nearly a month, her liver was enlarged by 3–4 centimeters below the rib margin, and her spleen was about 6–8 centimeters, with a smooth surface but slightly firm texture. Her white blood cell count was 32,000/mm³, with bloating and discomfort in the stomach, belching and acid regurgitation, pain in both flanks, a string-like, powerful pulse, and a thin white coating on the tongue. According to the lung-kidney deficiency and liver wood overcoming earth theory, the treatment was modified Xiaoyao San, adding 15 grams of Danshen, 9 grams of Guang Yu Jin, 18 grams of live oyster shell, 54 grams of live turtle shell, 9 grams of live pearl, 9 grams of live malt, 9 grams of peach kernel paste, 6 grams of safflower, 15 grams of calcined seashells, 3 grams of Jing Tian San Qi, and 9 grams of Pang E Shu, to dissolve stasis, detoxify, eliminate accumulation, and grind down the masses. After taking about 20 doses, her liver and spleen significantly shrank, her subjective symptoms improved, her white blood cell count dropped close to the normal range, and the dosage of the Qi-Nourishing and Yin-Tonifying Decoction was increased fourfold, with honey refined into pills. To date, it has been six years, and she has been engaged in physical labor for two years, with no recurrence observed.
[Case 2] Mr. Li, 36 years old, a soldier. August 4, 1976, first consultation: the patient was hospitalized and diagnosed with acute monocytic leukemia. After receiving 6-thioguanine, prednisone, and multiple blood transfusions, his condition improved somewhat, but a week ago he suddenly developed persistent high fever ranging from 39°C to 40°C, fell into a coma, refused to eat, and relied on intravenous fluids for sustenance. Blood test results: hemoglobin 13.4 grams, red blood cells 4.02 million/mm³, white blood cells 3,900/mm³, with 2% blast cells, 2% promyelocytes, 2% metamyelocytes, 3% segmented neutrophils, and 2% immature monocytes. Examination revealed a dark red complexion, a thin yellowish greasy coating, a floating and weak pulse, along with astringency, indicating lung-kidney yin deficiency and internal heat burning up, treated with a formula combining qi-nourishing and yin-tonifying with pathogen-eliminating effects. August 11, second consultation: after taking the above prescription for six doses, his temperature returned to normal, he regained consciousness, his appetite improved, the coating on his tongue disappeared, but he still felt shortness of breath and excessive sweating, occasionally experiencing palpitations, so the formula was adjusted by removing Qin Jiao and turtle shell ingredients, adding 6 grams of Qing Dai and 6 grams of Pang E Shu. March 29, 1977, 15th consultation: blood test results: hemoglobin 13 grams, red blood cells 4.42 million/mm³, white blood cells 5,000/mm³, with 55% neutrophils, 2% acidic, and 2% monocytes. Added 120 grams of Taizi Shen, 120 grams of Xuan Shen, and ginseng roots as Jilin ginseng, brewed into a paste, taken twice daily, morning and evening, dissolved in hot water. December 1977, after 16 months of remission, the disease suddenly relapsed, with a large number of primitive monocytes appearing in the bone marrow, far exceeding the normal range, immediately undergoing combined chemotherapy, but before completing even one course of treatment, the bone marrow was severely suppressed. January 21, 1978, 22nd consultation: blood test results: hemoglobin 8.5 grams, red blood cells 2.89 million/mm³, white blood cells 250/mm³, platelets 30,000/mm³. Examination revealed profuse sweating, shortness of breath, confusion, refusal to eat, a very weak pulse, so the formula was changed to Sheng Yang Jiu Yin Tang, taken once daily for five consecutive days, and the blood counts began to improve, so the original prescription was continued. March 8, 24th consultation: all symptoms completely subsided, everything felt normal, spirits improved, appetite normalized. Blood test results: hemoglobin 13.4 grams, red blood cells 4.52 million/mm³, white blood cells 6,800/mm³, with 83% neutrophils, 2% acidic, 1% alkaline, and platelets 126,000/mm³. Bone marrow examination: within normal range. The disease entered a second remission, and he is still taking the qi-nourishing and yin-tonifying paste, having now been in remission for 26 months, under observation.

Understanding of Leukemia in Traditional Chinese Medicine

The etiology and pathogenesis of leukemia have not yet been fully elucidated, but according to current research trends, the viral theory and the apoptosis theory have received widespread attention. The former posits that certain viruses, through the action of reverse transcriptase, can transform into leukemia under specific conditions.

The latter theory holds that dysregulation of the differentiation, proliferation, and maturation processes of leukocytes themselves can lead to the development of leukemia. Given that one theory emphasizes internal factors while the other focuses on external factors, many believe that both types of factors may coexist in the pathogenesis of leukemia. Therefore, some advocate that leukemia treatment should boldly move beyond the currently popular chemotherapy approach that primarily aims to suppress tumor cells, and instead focus on regulating and promoting normal cellular functions, fully mobilizing the body's own anti-disease mechanisms. This holistic perspective is entirely consistent with the traditional Chinese medicine principle of "strengthening the righteous qi to dispel the pathogenic factors."
I. The Relationship Between Leukemia and the Five Zang Organs
The Yuan dynasty scholar Zhu Danxi proposed: "Blood belongs to yin; it is difficult to form but easy to deplete." He further stated: "It is generated by the spleen, summarized by the heart, stored in the liver, distributed by the lungs, excreted by the kidneys, and irrigates the entire body."^①^ This illustrates the close relationship between blood and the five zang organs. He also said: "Blood belongs to yin, and yin is characterized by stillness; only when it remains stable and well-regulated can it harmonize with the five zang organs, circulate throughout the eight fu organs, and be properly contained within the blood vessels."^②^ In leukemia, white blood cells, as a major component of blood, exhibit extremely vigorous proliferation and activity, indicating that blood has lost its normal state of "stillness" and "containment." Consequently, this inevitably leads to disruption of the qi flow and functional disorders of the zang-fu organs. Therefore, the relationship between leukemia and the five zang organs cannot be ignored.
