Hepatitis B virus (HBV) mutations on September 24, 2002
Two unexpected situations arise during hepatitis B treatment: 1. Pre-C region mutation: three-system presentation of “small three yang,” i.e., HBeAg(-), yet HBV-DNA quantitative analysis shows significant replication, >1×10^4^ copies/mL, indicating that this individual’s HBV is a pre-C region mutant strain, requiring extended treatment duration. 2. YMDD mutant strain: if ALT remains persistently elevated after more than six months of standard treatment, various causes should be ruled out, then it can be considered a YMDD mutant strain. Both of these situations have poor prognosis, necessitating intensified treatment and prolonged therapy.
