Further Discussion on the Pathogenesis and Treatment of Ulcer Disease 1999.3.19
The second issue of the Chinese Journal of Internal Medicine in 1999 featured articles by Wang Shenggen and Zhu Renmin, both of which studied peptic ulcers (DU). Currently, it is believed that: ① Helicobacter pylori (Hp); ② gastric acid; ③ impaired gastric emptying are the three primary factors contributing to the formation of DU. Wang et al. separately used antibiotics, famotidine, and prokinetic agents to treat ulcer disease, with famotidine showing superior efficacy in alleviating symptoms and promoting ulcer healing compared to the other two, while antibiotics also demonstrated good symptom relief and ulcer healing, though not as effective as acid-suppressing drugs, and prokinetic agents proving only marginally effective. Therefore, it is evident that gastric acid plays the most crucial role in the formation of ulcers, followed by Hp, with impaired gastric emptying ranking third. However, these three factors are interdependent: when gastric emptying is slow, gastric acid secretion increases, and Hp also rises accordingly; conversely, when Hp increases, gastric emptying slows down further, and gastric acid secretion likewise increases.
