Case 5: Mr. Wu, male, 48 years old
Initial Consultation: January 22, 2018
Chief Complaint: The patient has suffered from chronic nephritis for many years, with poor results from multiple treatments. Following a recommendation from the patient, he came to Professor Pei Zhengxue’s clinic for treatment. According to the laboratory reports provided by the patient, urea nitrogen was 22 mmol/L, serum creatinine 380 µmol/L, urinary protein (+++), urinary occult blood (+), left kidney atrophy, elevated blood pressure, while other parameters were normal. The tongue was red with thick yellow greasy coating, and the pulse was弦滑.
Western Medical Diagnosis: Renal failure, azotemia.
Traditional Chinese Medicine Syndrome Differentiation: Kidney yang deficiency, damp-heat and blood stasis.
Treatment Principle: Warm the kidney and activate blood circulation to resolve stasis.
Prescription: Jisheng Shenqi Tang, Large Rhubarb Three-Component Formula (Professor Pei Zhengxue’s empirical formula) with modifications.
Ingredients: Rehmannia glutinosa 10 g, Atractylodes macrocephala 15 g, Cornus officinalis 10 g, Poria cocos 10 g, Cortex Moutan 6 g, Plantago seeds 10 g, Cinnamon 10 g, Aconite 6 g (decocted first), Plantago seeds 10 g, Achyranthes bidentata 10 g, Sanleng 10 g, Curcuma zedoaria 10 g, Rheum palmatum 10 g (added later), Bai Mao Gen 20 g, Bai Li 20 g, White Snake Tongue Herb 15 g, Astragalus 30 g, Salvia miltiorrhiza 30 g, Su stem 20 g, Cicada slough 6 g, Motherwort 15 g, Talc 10 g (decocted separately), Aristolochia fangchi 10 g, Magnolia officinalis 10 g, Akebia quinata 10 g, Acorus calamus 10 g, Panax notoginseng 3 g (added later), Leech 10 g (added later). Decoct in water and take five doses for significant effect. Urinary protein (-), urinary occult blood (-), blood pressure decreased. Continue using this formula with adjustments for two months until complete recovery.
