Input: Suo. The pathogenic toxin of scarlet fever is inhaled through the mouth and nose, causing the lungs and stomach to be affected by the toxin. When the heat is severe and the toxin is intense, it immediately transforms into heat and fire within the body. The throat serves as the gateway between the lungs and stomach; when the toxin and fire surge upward into the throat, redness, swelling, and burning pain occur, leading to tissue decay and pus formation—this is what is known as “when heat is excessive, swelling occurs; when toxin is severe, corruption leads to ulceration.” The lungs govern the skin and hair, while the stomach governs the muscles. When warm, toxic pathogens accumulate in the lungs and stomach, they spread outward through the skin, manifesting as scarlet rash that covers the entire body surface like fine threads. This disease progresses along the Wei, Qi, Ying, and Xue pathways. In the early stages, symptoms often present as manifestations of the lung-Wei system, though these symptoms are typically short-lived. As the pathogenic toxin moves from the exterior to the interior, it transforms into dryness and fire, filling the Qi and Ying channels, potentially leading to severe conditions characterized by both Qi and Ying being burned. For those with inherent deficiency in their vital energy, the pathogenic toxin may penetrate inward to the Jueyin, causing heart fire to become excessively heated, liver wind to stir, and the combined forces of wind and fire to exacerbate the condition, resulting in high fever, delirium, and convulsions. If blood heat becomes severe, it can lead to abnormal blood flow, causing purplish-red rashes or petechial hemorrhages, and even posing a life-threatening risk of internal obstruction and external collapse. In the later stages of the disease, heat consumes yin fluids, causing damage to the yin of the lungs and stomach; as the condition shifts from excess to deficiency, heat and toxin may affect the heart, spreading to the joints, or impairing the water metabolism functions of the lungs, spleen, stomach, and kidneys, leading to various complications. 1. Pathogen in the Lung-Wei Region The primary symptoms include sudden onset of illness, fever, headache, chills, redness and swelling of the throat, severe cases leading to ulceration, nausea and vomiting, sparse rash, a red tongue, a thin white or yellow coating on the tongue, a floating, rapid, and forceful pulse. Treatment focuses on clearing heat, relieving throat inflammation, and detoxifying the body, using a modified Yin Qiao San formula: 20g of Lonicera japonica flower, 20g of Forsythia suspensa, 10g of Radix Paeoniae Alba, 10g of Platycodon grandiflorus, 10g of Bupleurum chinense, 6g of Licorice root, 10g of Flos Sophorae, 10g of Herba Schizonepetae, 9g of Radix Paeoniae Rubra, 30g of Herba Lysimachiae. The herbs are decocted in water and taken once daily.
2. Pathogen Entering the Qi and Ying Channels The primary symptoms include high fever, irritability, thirst, and extreme throat congestion; tonsils are red and swollen, with white exudate; the rash is widespread and scarlet, the tongue appears red and dark, with papillae resembling bayberries, the tongue coating is yellowish and rough, eventually peeling off; the pulse is full and rapid. Treatment should focus on clearing the Qi and cooling the Ying, clearing heat and detoxifying the body, using a modified Qingwen Baidu Yin formula: 30g each of Radix Isatidis, Fresh Rehmannia Root, 20g of Gypsum, 9g of Cortex Phellodendri, 9g of Radix Paeoniae Rubra, 9g of Anemarrhena Asphodeloides, 10g of Scutellaria Baicalensis, 9g of Phyllostachys Nigra, 5g of Platycodon grandiflorus, 4.5g of Licorice root, and 3g of Coptis chinensis. The herbs are decocted in water and taken once daily.
3. Pathogen Toxicity Penetrating Deeply: The primary symptoms include high fever, irritability, coma, delirium, convulsions, purplish-red rash or accompanied by petechial hemorrhages, a red and dark tongue, and a fine, rapid pulse. Treatment should focus on clearing the Ying and cooling the Blood, calming the mind and extinguishing the Wind, using a modified Qingying Tang formula: 10g of Rhinoceros Horn, 30g of Fresh Rehmannia Root, 30g of Purple Herb, 9g of Lonicera japonica flower, 9g of Forsythia suspensa, 9g each of Cortex Phellodendri and Radix Paeoniae Rubra, 3g of Coptis chinensis, 9g of Phyllostachys Nigra. The herbs are decocted in water and taken once daily.
4. Remaining Toxins Still Present: The primary symptoms include fever, palpitations, chest tightness, fatigue, excessive sweating, joint pain, a thin tongue coating, and a rapid, weak, or irregular pulse. Treatment should focus on replenishing Qi, nourishing Blood, and nourishing Yin while calming the Heart, using a modified Zhigan Tang formula: 15g of Licorice root, 10g of Ginseng, 10g of Angelica sinensis, 15g of Salvia miltiorrhiza, 12g of Rehmannia root, 15g of Ophiopogon japonicus, 10g of Dendrobium officinale, 10g of Schisandra chinensis, 10g of Cassia seeds, 5g of Cinnamon twig. The herbs are decocted in water and taken once daily.
5. Yin Deficiency in the Lungs and Stomach: The primary symptoms include loss of appetite, fatigue, dry lips and mouth, or dry cough, flaky skin, a red tongue with little saliva, and a fine, rapid pulse. Treatment should focus on nourishing Yin and generating Fluids, clearing heat and moistening the throat, using a modified Shangbiao Dongtang formula: 20g of Saishen, 10g of Tian Dong, 15g of Ophiopogon japonicus, 15g of Dendrobium officinale, 15g of Radix Paeoniae Rubra, 10g of Platycodon grandiflorus, 6g of Licorice root, 10g of Reed root. The herbs are decocted in water and taken once daily. If the patient is irritable and restless, or if the tongue is red and covered with papillae, add 10g of Bamboo Leaf, 9g each of Radix Paeoniae Rubra and Radix Paeoniae Rubra; for patients with dry throat and a desire to drink, add 12g of Honeysuckle Flower and Dendrobium officinale; for constipation, add 9g each of Rheum palmatum and Radix Paeoniae Rubra; for high fever and delirium, administer An Gong Niu Huang Wan. Sweat-Inducing Methods (Divided into Four Steps) ① The Jie Ji Tu Sha Tang formula is specifically used for early-stage scarlet fever, treating symptoms such as chills and fever, redness and swelling of the throat, body aches, and discomfort with nausea and vomiting. ② The Ma Xing Shi Gan Tang formula, with modifications, is used for cases where the scarlet fever rash does not fully erupt, with cold chills and fever, white or foul-smelling discharge in the throat, or severe cases of coughing and respiratory distress. ③ The Jia Jian Ge Gen Sheng Ma Tang formula is used for cases where the rash is widespread but the head and face remain unaffected, with fever and no sweat. ④ The Bai Du San formula is used for cases where the scarlet fever rash has not yet fully erupted, leaving behind scarlet toxins, swelling, hardness, and painful sensations, with fever and no sweat. 2. Clearing Methods (also divided into Four Types) ① The Jia Jian Hei Gao Tang formula is used for cases where the pathogenic toxin fails to reach the target area, causing dryness and depletion of bodily fluids, with scarlet rash but no full eruption, fever without sweat, redness and swelling of the throat, white discharge, and thirst and irritability. ② The Liang Ying Qing Qi Tang formula is used for cases where the scarlet fever rash is widespread, accompanied by high fever, thirst, and a desire to drink, even with delirium and verbal hallucinations, swollen and discolored throat, a rapid pulse, and a red or dark, lacking moisture tongue. ③ The Jia Jian Zi Yin Qing Fei Tang formula is used for cases where the scarlet fever rash is widespread, with fever and no improvement, thirst and a desire to drink, or with sore throat, a feeling of heaviness in the throat, and excessive phlegm and cough. 3. Detoxification Methods (also divided into Four Types) ① Use large amounts of bitter and cold herbs to directly induce diarrhea, or combine with saltpeter to use salty, cold, and bitter herbs to promote bowel movements; alternatively, use the Liang Ge San formula to clear the exterior while attacking the interior, or use Chen Jin Zhi to clear the exterior and attack the interior; or use Chen Jin Zhi to clear the exterior and attack the interior. In addition to medicinal treatments, there are also external therapies that can complement oral medications. ① For severe throat swelling, slice white-edged Yunnan green tea roots and hold them in the mouth (with strong cardiac and detoxifying effects). ② Crush garlic and soybeans into a paste, wrap it around the tiger’s mouth area about two inches away from the wrist bone, and when bubbles form in the paste, squeeze out the liquid and apply the paste, ensuring it does not come into contact with dirt. ③ Use gold not replaced to blow into the throat (add three qian each of white-green tea and black tea, three parts of rhino horn, grind them finely and blow into the throat). ④ Use the “Ten Treasure Powder” formula to blow into the throat, adding Western Yellow Cattle, large mother-of-pearl (remove the core), and horse hoof fungus, each three parts; calcine human middle ear bones, grind white amber and fish bile (large ones dried in the sun, or if unavailable, use three dried green fruit cores as substitutes for charcoal). Add five parts each of large pearls (boiled in tofu to remove oil and then ground into six parts); half a piece of large plum is not suitable for use if the lesion is already ulcerated. Add four parts each of borax and human nails (both sides of the body, though they may be difficult to see when roasted for a moment, use four parts of calcined dragon bone instead). Grind all ingredients finely and use them exclusively for cases where lesions have already formed or are still forming, with redness and swelling of the throat, as well as for treating smallpox toxins, post-smallpox tooth decay, and after pomegranate poisoning, to reduce swelling, relieve pain, and promote tissue regeneration. Old Chinese medicine practitioner Ren Yingqiu at Beijing College of Traditional Chinese Medicine had a detailed approach to differentiating and treating scarlet fever: ① During the prelude stage, use the Jie Ji Tu Sha Tang formula to treat early-stage scarlet fever, addressing chills, fever, throat swelling and pain, difficulty swallowing, and general body aches. Symptoms include irritability and nausea. ② During the rash stage, use the Qingre Touzhen method to treat cases where the body temperature rises and the throat hurts, with red rashes all over the body. ③ During the shedding stage, modify the Shengma Ge Gen Tang formula to treat cases where the scarlet fever rash is widespread, but the head, face, and nose remain unaffected, with fever, diarrhea, and sore throat without ulceration. ④ The Liang Ying Qing Qi Tang formula is used for cases where the scarlet fever rash is widespread, accompanied by high fever and irritability, thirst for cold drinks, even with delirium and verbal hallucinations, swollen and discolored throat, a rapid pulse, and a red or dark tongue lacking moisture. Old doctor Chen Cunren treated scarlet fever with his first formula (days 1–3): The treatment should focus on inducing sweating, using the Jie Ji Tu Sha Tang formula. The second formula (day 4) emphasizes clearing and resolving the rash, using the Qingwen Baidu Yin formula. The third formula (days 4–5) was used for cases where scarlet fever worsened, employing the Xiangjiao Di Huang Tang formula. The fourth formula (after recovery) was the Sangju Yin formula. Yao Fengwu believed that scarlet fever belongs to the category of “warm-heat epidemics,” and should be differentiated according to the patterns of Wei, Qi, Ying, and Xue. For mild cases, use the Yin Qiao Jie Du Tang formula with modifications; for surgical cases, use the Huanglian Jie Du Tang formula with modifications; for severe cases where Qi and Ying are both burned, use the Qingwen Baidu Yin formula with modifications, taking An Gong Wan; for cases where the rash does not fully erupt, add Kegeng and Mint; for high fever, add Gypsum, Zisu Dan; for purple and black rashes, add Danpi and Red Peony, large rhino horn, and for delirium, add Cicada Shell and Gou Teng; for sore throat, use the Yu Ya Quan San formula to blow into the throat. For constipation and abdominal distension, add Rheum palmatum and Mingfen; for sore throat, add Liu Shen Wan; for toxic myocarditis due to poisoning, add Yu Zhu, Huang Jing, Mai Dong, raw Mountain Yam, raw Oyster, raw Turtle Shell, raw Turtle Plate, etc. For purulent toxicity cases, add Lonicera japonica flower, Public Echinacea, Dandelion, Wild Chrysanthemum, and Tian Kui Zi; for thirst, add Honeysuckle Flower; during the shedding stage, use the Sha Shen Mai Dong formula frequently. For Scarlet Fever, in the early stages, it is important to clear the lungs and Wei, allowing the pathogenic toxin to move outward, penetrate the interior, and spread to the Qi and Ying channels; treatment should focus on clearing heat and detoxifying, clearing both Qi and Ying, and transforming the Qi back into the Yang. If the pathogenic toxin penetrates the Ying and Blood channels, treatment should focus on clearing the Ying and cooling the Blood, clearing heat and detoxifying. ① For cases where the pathogenic toxin resides in the Lung-Wei region, treat with pungent, cool herbs to clear the toxin and relieve throat inflammation, using the Yin Qiao San formula. ② For cases where the pathogenic toxin enters the Qi and Ying channels, treat with Qi and Ying cleansing, clearing heat and detoxifying, using the Qingwen Baidu Yin formula. ③ For cases where the pathogenic toxin penetrates the Ying and Blood channels, treat with clearing the Ying and cooling the Blood, clearing heat and detoxifying, using the Qingying Tang formula. ④ For cases where the pathogenic toxin affects Yin and causes damage, treat with nourishing Yin and strengthening Qi, while also clearing residual toxins, using the Zeng Ye Tang formula with modifications. Single-herb treatments: ① 10g of Horse Hoof, 10g of Lonicera japonica flower, 15g of Indigo Woad root, ground into powder and taken orally. ② 15g each of Earth's Cowpea and Indigo Woad root, decocted in water and taken. ③ 15g of Indigo Woad root, 30g of Big Blue Leaf, 6g of Licorice root, decocted in water and taken. ④ 15g of Andrographis paniculata, decocted in water and taken. Wang Qin used a combination of traditional Chinese and Western medicine to differentiate and treat scarlet fever based on clinical manifestations. If a child suddenly develops fever, headache, chills, redness and swelling of the throat, severe cases leading to ulceration, nausea and vomiting, sparse rash, a red tongue, a thin white or yellow tongue coating, and a floating, rapid pulse, this indicates that the pathogenic toxin is present in the exterior. The treatment should focus on clearing heat and detoxifying the throat, using the Lonicera japonica flower, Forsythia suspensa, Radix Paeoniae Alba, 10g each of Platycodon grandiflorus, Horse Hoof, 6g of Licorice root, 10g of Flos Sophorae, 15g of Flos Sophorae, 10g of Platycodon grandiflorus, 30g of Big Blue Leaf. The herbs are decocted in water and taken once daily. If the child presents with high fever, irritability, thirst for drinking, extreme throat congestion, red and swollen tonsils with white exudate, widespread scarlet rash, a red and dark tongue with papillae resembling bayberries, a yellowish and rough tongue coating, followed by peeling, and a rapid pulse, this indicates that the pathogenic toxin is present in the interior. The treatment should focus on clearing the Qi and cooling the Ying, clearing heat and detoxifying.
