Hemolytic anemia (HA) on January 27, 2003

HA is generally divided into intravascular hemolysis and extravascular hemolysis, with causes further categorized as intracellular abnormalities and extracellular abnormalities. ① Intracellular abnormalities: hereditary spherocytosis, paroxysmal nocturnal hemoglobinuria, glucose-6-phosphate dehydrogenase deficiency. ② Extracellular abnormalities: β-lipoprotein deficiency (spiky cell syndrome), chemical substances, physical factors, and hemolysis caused by hyperfunction of the spleen. Characteristics of HA: ① Increased free hemoglobin (>4%); ② Increased serum iron >21 µmol/L; ③ Increased iron granulocytes in bone marrow (+ to +++); ④ Positive occult blood in hemoglobinuria; ⑤ Increased bilirubin; ⑥ Increased urobilinogen and urobilin; ⑦ Reticulocyte increase >1% to 3%, reaching up to 30% in severe cases. Single remedy for HA: 15 g of saltpeter, 30 g of black alum, 30 g of turmeric, 30 g of danshen, ground into powder, made into pills of 9 g, taken three times daily with yinchen water.
