Discussion on the Treatment of Liver Pain 1999.4.7

Liver pain is the most common clinical symptom among patients with liver disease and also one of the most challenging problems that must be addressed. Western medicine offers no specific cure for this condition, whereas Traditional Chinese Medicine demonstrates remarkable efficacy, making it the primary reason why patients with liver disease seek treatment from TCM practitioners. I believe that liver pain is primarily caused by liver enlargement, which compresses the nerve endings of the hepatic capsule; it can also result from obstruction or infection of the intrahepatic bile ducts; additionally, degeneration, necrosis, and regeneration of fibrous tissue extending toward the hepatic capsule contribute to the pain. All three of these causes are accompanied by bitter taste in the mouth, dry throat, and irritability, which Traditional Chinese Medicine classifies as stagnation of liver qi. When liver qi stagnates, it transforms into fire, and qi stagnation invariably leads to blood stasis. Stagnation of qi and blood stasis, coupled with liver qi transforming into fire, constitute the primary pathogenic mechanisms behind liver pain. Therefore, promoting qi circulation and activating blood flow, as well as clearing heat and draining fire, are the two fundamental principles for treating liver pain.
