Patient Zhang, male, 20 years old, admitted with a diagnosis of systemic lupus erythematosus (SLE). Initially presented with acute hepatitis, with bilirubin levels exceeding 120 mmol/L during the severe phase, showing a tendency toward liver necrosis. After treatment, although the above-mentioned symptoms gradually improved, macroscopic hematuria once occurred; following treatment, the hematuria subsided, and the erythrocyte sedimentation rate decreased from 121 mm/h to 7 mm/h. Subsequently, widespread systemic bleeding and massive epistaxis developed. Hemostasis with oil gauze packing in the anterior nasal cavity proved ineffective, so oil gauze packing was applied to the posterior nasal cavity for hemostasis, along with intravenous infusion of 6 g of ε-aminocaproic acid, 0.6 g of tranexamic acid, and 8 g of hemostatic agent, after which the bleeding stopped. No sooner had one crisis been quelled than another arose: extensive necrosis of the pharynx, perforation of the nasal septum, and ulceration of the hard palate were subsequently discovered. After more than ten days of intensive resuscitation, the patient’s condition finally stabilized. The fundamental medication for treating this disease is the Xiaofeng Chushi Dan formulated by me, two pills each time, three times daily. Gu Sheng I is taken at one pill per day, three times daily. Gui Zhi Shaoyao Zhimu Tang combined with Maxing Yigan Tang, Dang Gui Liu Huang Tang, and 15 g each of Chuan Wu and Cao Wu (decocted for one hour first), along with biological preparations such as transfer factor and thymosin—3 mg weekly for the former and 10 mg daily for the latter—as well as prednisone at 50 mg daily, administered as a single dose. The Xiaofeng Chushi Dan is prepared by decocting Chuan Wu and Cao Wu, then combining it with Gui Zhi, Bai Shao, and Zhimu; this is the fundamental medicine for treating connective tissue diseases and can also treat nephritis and various joint disorders. Previously, many cases of such illnesses have been cured using this medicine. In this particular case, the patient was able to overcome numerous obstacles and achieve final remission thanks to the combined effects of the aforementioned medications; among them, however, Xiaofeng Chushi Dan plays the leading role. Blood transfusion can buy time, allowing the traditional Chinese medicines to take effect. Finally, the patient developed necrosis in the pharyngeal region, which is entirely consistent with Wegener's granulomatosis as I have observed. According to my observations, Wegener's granulomatosis commonly affects young and middle-aged adults, often accompanied by malignant lymphoma or malignant reticulosis; it appears like a malignancy but is not truly malignant, has a high mortality rate, is difficult to treat, and falls under the category of connective tissue diseases.
